Erm..."cotton eyed" usually either means drunk, or that your eyes [for whatever reason] have turned milky white in the pupil.

So basically it's saying either the drunk and/or blind guy is the reason [the narrator] hasn't gotten married. [I've always just assumed he was such a lech he drove all the women away from the town.]
