Uh...yes. They do do that. It is called "AU". Or crossovers. If it just a random OC (ex. Sherlock has a case with some OCs), no they shouldn't mention it. BUT if the OC is in a romantic relationship with a main character OR if the OC is a main character, it should be mentioned.

There is definitely a difference, yes. If the OC is the focus of the fic, you should mention that. I think you and the anon up there are talking about two different uses of OCs.

That being said... I wonder what your opinion is on characters that were mentioned but never met in canon. Do they count as OCs since we don't know anything about them and the author is building them pretty much from the ground up? (ie Coulson's cellist)

Most people don't consider characters like that OCs. Heck, I have seen people tag "The Cellist" (he/she has a character tag on Ao3). While they are pretty much an OC, a name gives them a character. I would assume most people figure a character like that is basically an OC, though.
