That makes sense. He's seen as smart and educated so it's assumed he might know a thing or two, or at least be able to muddle through. For all we know, people were coming to him asking for help with financial problems, leaky roofs, sick animals, relationship issues, etc.

He tells Tony he's "Not that kind of doctor" because Tony has the resources to get real counseling.
