Isn't that Russia's entire shtick? Being a creepy megalomaniac who hides behind an innoccent fassade? I'm not that deep into Hetalia fandom but even the characters in canon are constantly afraid of him, creeped out and fear him. How can this go unnoticed?

Pretty much. Though part of it's also because the creator said...somewhere [I can't remember where] that Bloody Sunday drove Russia legit crazy and that does sometimes get touched upon.

Technically he has a "childlike innocence", which would also include a childlike cruelty I guess? Canon is that he just wants friends or w/e and doesn't realize that "friends" aren't the people you beat up and drag home to live with you.
