He's technically based on a character with that name-- the series is Brave10 and is loosed based on the legend of the Sanada Ten Braves from the Warring States period of Japanese history. The majority of the character names are taken from that. And considering the character, it's perfectly likely that he wasn't born with that name and gave it to himself.

Yeah, but when you have a manga/anime character of ambiguous gender with "kama" in the name there's a good chance they're referencing "okama," like Rurouni Kenshin's Kamatari. It's a pretty blatant name pun.

And apparently he also fights with a scythe!

I don't think the author was necessarily going for that pun, though. Like I said, Shimotsuki Kairi didn't make up the name, it's from a well-known legend. The original character of Yuri Kamanosuke presumably was not ambiguous gender at all (and as the wiki page notes, another series that uses the ten braves makes Kamanosuke a woman). The only names she changed from the original legend were for the two female characters and part of Benmaru's name (because his given name is the same as one of the others). There was going to be a Kamanosuke in the group no matter what, pretty much. I'll concede it's possible she decided to take the pre-made name and go for the 'okama' pun, but it's also perfectly possible she just wanted to make a cool-looking guy wielding a scythe and started having fun from there.

I get that the name came first, I'm just saying that when a creator chooses to make "Kamanosuke" a scythe user of ambiguous gender, the default assumption ought to be that she's playing on the obvious pun, and that he's a cross-dressing man.

But he's not a cross-dresser. The only point in the series he actually cross-dresses he's under the influence of an illusion spell. The running joke is that other people think he looks feminine and mistake him for a girl, but his style of dress isn't any more feminine than any of the other guys in the series (seriously, if any of the boys are shopping in the ladies section of the Improbable Ninja Wear Outlet, it's Rokuro and his super-low rise pants that show off his magical hips). He's not an okama character.
