Maybe the lack of smut is due to so many of the characters being underage by our standards? Then again, that hasn't stopped other fandoms, so I really don't know what it is.

The age doesn't seem to be a problem. I think it's just hard to imagine a lot of these characters being really KINKY. There's smut, but mostly vanilla smut. Maybe the medieval setting makes it harder to go for hardcore kink and BDSM scenarios and people having extensive discussions about what they're into and safewords and all that stuff. That and many characters don't strike you as the kinky type. I mean, people like Ned or Robb or Sansa or Stannis or Jon would probably be somewhat hung up about anything other than missionary.

That's what modern AUs are for!
But yeah, I can only really imagine kinky Boltons and Ironborn scenarios (like other posters have said). Maybe kinky Freys too!
