At the point where their paranoia starts affecting other people more than them.

These women appear comfortable with their paranoia enough not to notice it, so there's no point in telling them to fix it. You can just scroll.

Pretty much this, with an elaboration as to how much it interferes in their own life, as well. It's one thing to be convinced that every man sitting down next to you on a bus is out to feel you up or rape you, but ultimately just sit there and do nothing about it and most likely go unmolested. It's quite another to consistently get up or move, even if there are no other seats available and you inconvenience other passengers in the process, or to not use public transportation at all in the first place due to this paranoia, even though doing so would be extremely costly and/or efficiency ineffective for you or the people around you.

Yeah, this. Unless these people are friends and the screeds all end up in your inbox, just scroll. Telling them to get help just reads like concern trolling to me. If they're that annoying, unfollow them or tumblr savior their rants.
