Kim Possible and Shego are half-sisters. James Possible's high school girlfriend moved away without ever telling him she was pregnant, and her family discreetly adopted the baby out to a couple who wanted a third child but had been advised against trying to conceive one. Their second child had been born with significant health problems and required constant care for the first couple years of his life and more attention throughout his childhood, hence Mego's self-centeredness after spending his formative years in a world that really did revolve around him a lot of the time. Hego is the only one of the kids who knows that Shego was adopted, and while he considers her no less family than any of his brothers it contributed to his desire to shepherd her and make sure she was never excluded or left alone -- even when she wanted to be. The twins were a midlife surprise to their parents, born surprisingly healthy and dashing Shego's hopes of having a sibling she could actually bond with (and maybe push around a little) when it turned out there were two of them and they were happy enough with each other that they didn't "need" her.

I like this theory because the first time I watched Kim Possible, knowing nothing of the premise of the show, I thought Shego was supposed to be Kim Possible's evil twin, genetically engineered by Dr. Drakken or something, because they looked so similar to each other except for the coloring, to my newbie eyes.
