It's the context, really. There are a lot of different implications that go with "dog". They are wonderful, sweet animals and the best friends people could ask for. If you're my friend and I like you as much as I do my dog, that's a huge deal and you've apparently earned a lot of my trust. But people aren't dogs, and if someone is comparing a person to a dog, it means they are drawing a parallel to a specific thing. It's usually either inequality or blind faithfulness. Neither is a good thing. People who use this comparison aren't really trying to insult dogs with it normally, I don't think.
