Some people find that it's the things Hannibal is saying more than Mads' accent or cadence or what have you that make him hard to understand in the show. Like the one line that really stumped me was him talking to Will about "the bone arena of your skull" which is such an oddball thing to say that it's hard to recognize as real words at first, lol. They're using a lot of narrative stuff from the books as dialogue.
