The companions aren't always women (though they are usually women, and if there's only one companion it's almost always a women).

"The doctors" aren't always men; the Doctor is a single person who, to date, appears to be male. The show has the option of giving the Doctor a female regeneration, and may do that at some point in the future, but it would raise questions about Time Lord gender I'm not confident would be addressed very well.

They are always men, what are you saying? They don't HAVE to be, but they are.

It seems like the Doctor doesn't 'HAVE' to be male, biologically speaking; there was a mention of another member of his species who would be sometimes male and sometimes female, and the Doctor has expressed some unease on regeneration that he might end up female, but on the whole most Time Lords seem to stick to one gender throughout their different incarnations.

Personally, I don't want a female Doctor any more than I want a non-british one; the series changes so much, some continuity is quite comforting! lol

I think what anon above is saying (at least in part) is that there is no 'they'- despite changes of face and personality traits, the Doctor is one continous entity.

Ergo, it's not 'they are always men' so much as 'he is always a man'. Which, admittedly, he doesn't actually have to be, but it's not quite the same discussion.
