It's funny that you bring up the "the usual teenaged rapist/victim high school trope", because that's about the last thing I think of when I see the term hurt/comfort. I always think of it as referring to physical injury/illness. Emotional trauma I tend to categorize as angst, and non-con is an entirely separate category.

While I've been in fandom forever, I haven't been active in any particular fandom recently and mostly hang out here. So maybe the categories have changed somewhat.

That's the traditional "thing". It might have evolved a bit more these days, but as far as I was aware that's still a huge chunk of the genre.
