I've actually wondered this too, about police procedural type shows where the characters generally go by last names (but not always). From what I've observed in the Law & Order fandom, it seems to relate to how "close" the fandom feels to the character, if that makes sense.

Like, in the original L&O not much is known about the characters' personal lives, so they're mostly referred to by last name in the fandom (with some exceptions). In SVU they're mostly referred to by first name except for Cragen, whose personal life wasn't mentioned nearly as often as the rest of the characters' until this season, and Munch, who is generally known by his last name onscreen. The new characters were called by their last names in fandom until they'd been there for a while, which is why I think it has to do with how well the audience "knows" them.
