It depends on how far into her career a geisha is. When she first starts out as a maiko (apprentice geisha), she usually just paints the bottom lip, then gradually begins to fill in the top as she gets more experience.
Or at least that's how it works in Kyoto nowadays.
In the book, I remember Sayuri saying even Hatsumomo only painted the bottom lip, but then the movie took artistic liberties with the hair and makeup.
