Floss. Floss is the best tooth cleaning invention there is. Just take a piece of floss and make sure to get the back and outer sides of the tooth in addition to the crack between it and the tooth in front of it.

yeah i floss every day, but the space back there is so cramped that it's hard to position my fingers in a way that makes for easy cleaning on the troublesome side of my molar. :(

Have you tried the flossers that are basically little forks with a bit of floss between the arms? I have a small mouth and similar flossing difficulty, and these things help so much.
