nayrt, but I have a similar thing, and for me it's a bit of both? I can't really explain why, but definitely the 'hurt' part of strikes a strong reaction - and these are always characters that I love, and I put them through the wringer. I think the flip side is the 'comfort' part, but that's not the part that gets the visceral reaction. In fact, I don't like fics that dwell too much on the long slow recovery part, but I do like fics that dwell on the trauma of the injury. And I have no idea why - it's just a thing with me.

This has just started happening to me lately too. I was surprised at first, but I guess I really do like putting my favorite characters to the test. It's fun.
