I was told (by a couple different doctors) that if you're not having problems and you're not sexually active, you don't need one, regardless of age.

As far as "can I say I don't want to do it?" you absolutely can. You can refuse any medical procedure and a doctor can't force you to do it. They might try to pressure you to do it, but you can still say no.
