I don't mind photorealistic fanart as a rule-- Euclase does beautiful digital paintings that could cause a double-take, usually a single-subject portrait from one fandom or another, and they're just stunning.

Manips on the other hand... A little 'shopping to move character A closer to character B I can enjoy as something light and 'fun', but those fandom actor head on porn actor body things always squick me out, and that was the HUGE trend when I was first online. Or a digital paint-over of a porn actor to look like a comic book character (these were the days before the X-Men had a movie, after all)... always way in the uncanny valley, and always one of those things that was just kind of cringeworthy.
