I think the reason it was just a big twist to so many people is because they just weren't paying attention. The older brothers line was said in casual conversation, so many audience members probably just thought "Ah, just some backstory. I'll just ignore that since it's probably not important to the story." The subtle hints in the song aren't very noticeable either, since the song itself just seemed to be "awkward." His movements and actions could easily be passed off as humorous and unimportant. The genre-saavy viewers were the ones who picked up on these things, not the casual moviegoers.

When you're aware of the twist and watch the film a second time, it's very easy to pick apart all of the little hints. First-time viewers aren't aware of them, so they aren't really suspicious. The moment they do become suspicious, though, Hans' act falls apart. It's just a matter of when/if they catch on.

Yeah. If you're not looking for it, all the little "pretending to be exactly what Anna wants" moments look more like "he's just kind of dumb and socially weird."
