It's already been uncovered that guns and other more modern technology have been developed repeatedly by different people, but been squashed and covered up by the government. So the reason the SNK world (at least what we can see of it) looks medieval has been deliberately stagnated at a medieval stage.
My theory is that the SNK world is a lot like the world of From the New World, where civilization got to a certain point before a catastrophic event (Telekinetics in FtNW, Titans in SNK) caused it to collapse and start over, so the story is actually taking place in the future.
I'm hoping the Titans are actually a very advanced biological weapon from 2000 years before the story's start that went seriously wrong and in the chaos everyone lost their history and most of their technology (aside maybe from the ruling class behind that religion, who clearly have a hand in keeping the masses ignorant) as they struggled to carve out territory with walls.
