First? The quadrants are supposed to originate with an alien species, for whom quadrant romantic relationships are as natural as human romance is to humans.

--All four are considered romantic, but don't all have to do with sex. The sex quadrants (heart and spade) are described as concupiscent, versus the conciliatory diamond and spade.
--The three-person one, the clubs quadrant, is the hardest to understand and makes the least amount of sense. However, it's not OT3. It's two people who are both sexually attracted to each other and hostile to each other, and a third person who steps in and keeps things sex-free. This is still considered romantic.
--It's a vocab thing. The human relationships most similar to the diamond and clubs quadrants are considered platonic, but all troll quadrant relationships are considered romantic.
--One way of looking at it is a sexy rivalry. Or, people who hate each other but are also obsessed with each other. It's not a healthy relationship by human standards, but trolls are really weird.
