I think the enjoyment of seeing male characters in pain also might be tied into why the hurt/comfort genre is so popular. People very rarely enjoy having a character slowly and brutally tortured to death. Now, there can be prolonged and gruesome suffering, but usually it seems to me that this is done so that the male character in question can then be rescued, healed, and tended to by another character. And the deeper the pain, the more intense the comforting that is required. For a lot of people, I think it might function as an emotional high. Just a guess here.
