Yeah, constant blinding migraines turn the best of us into rapists and/or brutally sadistic killers.

I think it's implied in the books that the reason for the migraines (and possibly his behavior) is a traumatic head injury. Not that it's a complete excuse for being a rapist and a murderer, but there's a little more to it than migraines. Major personality change after certain types of head injuries are a real thing, albeit not as exaggerated as it is in this example.

It can't have been that major a personality change since he shoved his brother's face in a fire as a child.

It's actually not known whether his headaches are due to some side effect of gigantism or something similar, or whether it's due to a head injury. One character's comment implies that it can happen as a result of the latter, but given that his behaviour in childhood was as brutal as it is in adulthood, I'm inclined to suspect its the former, rather than the latter, in his case.

Given everything we've seen of Ser Gregor, he's always been a complete monster.

OP just feels bad that he has migraines. It doesn't mean that she or he thinks the migraines caused him to be the way he is.

Yeah. I feel bad for book!Tywin in that, like me, he shaves his head because he's self-conscious about male pattern baldness. That's a frustration I understand and share. It doesn't mean that I don't think Tywin isn't a vile, dishonest, piece of shit. It just means I share one of his problems, so I empathize with that.
