And that'd make sense for someone who's got OCD tendencies, but it's completely out of character for Sherlock.

Yeah, not really. He might not have actual OCD, but he's always doing something, is bored easily, and fooling around with the napkins, making fussy little creases and whatnot would definitely give his hands something to do while his brain does other things.

Seems perfectly in character to me.

Also, he's fixating on a tiny detail that isn't important, which is the flip side of what makes him an awesome detective.
