The thing that bugs me is when anyone in the media or fic writers use "Sherlock and Watson" to refer to the two of them (you never see Holmes and John, but that would bug me, too). I don't have a problem with characters in adaptations doing that, because there might be instances where people are on a first name basis with one of the pair but not the others, but it irks me when people in the real world don't use either both last names or both first names to refer to them.
