THIS. Or when I don't follow who replied to whom properly. I once accidentally started a bunch of wank because I thought someone was responding to me, and it got really convoluted from there. Finally someone was like "why is the secret poster denying everything they said in their secret?" And I went "... oh crap." *Headdesk*
