I had to get a gov't security clearance about a billion years ago and before the internet as we know it.

Basically, anything that someone else could blackmail you for could cause you to fail the background check: an affair, being gay, writing porn under a pseudonym -- I'm sure there was a lot more. But the more public you were about whatever they'd dug up, the less likely you were to be blackmailed; I've heard that could make it a non-issue for at least the lower security levels.

So, could someone who is fandom and is open about still maybe fail to get higher levels of clearance?

Maybe, but you could fail for lots of things.
