Eeeh... by the time people are so far gone to believe in obvious make-believe, they usually are a little more non-directional gibbering messes, and a little less pulling off pre-planned murders.

It's not like it was a super elaborate plan. There were woods nearby and they went there, like many kids do to play. Originally they were going to try to kill her in one of the girl's houses, so this plan wasn't a masterpiece by any stretch.

The 'ringleader' also apparently believed she could communicate with Voldemort, and other such things. It just doesn't seem like the lawyer had to reach too far to realize the girl had mental problems.

I dunno. You can hear voices and plan a murder because the voices tell you to.

At least one of the girls has been found competent enough to stand trial.

Well, to be fair it wasn't very well executed. They had two false starts and then failed to actually kill their victim. Isolating someone and then stabbing them isn't that complicated of a plan.

I don't think that's necessarily true, though. There have been higher profile murders where the killers were deeply involved in a fantasy world:

Bludgeoning someone to death with a brick in a sock takes some planning, but not much, nor is it particularly sophisticated. Much like stabbing a friend to death.

Not to mention that if people are lost in "obvious make-believe" and yet not a gibbering mess as you mentioned, how would we know it? You'd only see the cases that confirm your supposition. Quite a few serial killers are under some wacky delusions, but they're not always caught right away because they function at an acceptable social level long enough to carry out their plans.
