My mom has it in her forearms, but she's also started getting arthritis in her hands and hips. I think the pain in her arms is from an injury to her spinal chord about 20 years ago, which bruised the spinal chord right around where the nerves running into her arms all. There are tons of theories around what might "cause" fibromyalgia, including spinal/neck injuries, but nothing confirmed.

It's kind of like the laryngitis (literally "swelling/inflammation of the larynx") of chronic pain: we know something's wrong 'cause you're in pain/inflamed, so here's a generic term to mean something and get you help.
