I flatly don't believe the reasons people cite for hating Amanda Palmer. Don't like her music? Think her Twitter presence sucks? Don't care for how she treats musicians? Actually managed to gin up some outrage for that Evelyn Evelyn thing? Sure, fine, those are all reasons to dislike someone.

But none of them are reasons for the pure, unadulterated HATE she gets, mostly from female fans. The hatred is completely out of proportion to anything she's said or done, and always phrased in a way that has nothing to do with the ostensible reasons for not liking her.

My leading theory is that it's all the fangirls who had crushes on Neil Gaiman, upset that she gets to suck his dick and they don't. I'm very open to other theories, though; that one's mostly a placeholder for now.

It's the suicide-faking, partner-traumatizing, using-said-trauma-as-"art" aspect of AP that I object to. And that she still seems to think this was an a-okay thing to do. That is flat-out loathesome behavior.

And in conjunction to a large amount of smaller offenses (some of which you mentioned-- also things like simulated rape on stage) adds up to say, yeah, I hate her and her actions. There's documentation for all of this, it's not like it's some libelous rumour!
