A part of me is terrified this is about me...in any case, I doubt you've done anything wrong. If she leaves you for not changing, then I'm so sorry, I've had that happen too, but you'll find people who care about you for who you are and won't leave you no matter how much you tell them you're scared they will, or how slow you are to change, because some of us just don't change easily or quickly and it's very hard, and there are people who will understand that. But please, don't just "have an accident" and disappear. I don't know who you are but I suspect she'll miss you and always wonder, and regret not being able to help you more.
