Fallout: New Vegas gets my vote, personally. The map is gigantic, and there's a million things and places to discover in it. Plus if you play in "Hardcore" mode, it really changes up how you think about where you are and where you need to be, by making you pay attention to your hunger, thirst, and tiredness. It's awesome.

+1, though really any of the Bethesda open world games works for this. If doing needs [thirst/hunger/rest] is your jam then there's a mod for most of their open world games that does that - and actually that's why Bethesda decided to just go ahead and include in FO:NV, iirc.

Fallout 3, with its lessened emphasis on story, is far more sandboxy, to be fair -- and FONV's "hardcore mode" is kind of broken. However, Project Nevada fixes and expands on the idea, and Fallout 3 has Fallout Wanderer's Edition, which is by far one of my favourite mods for any game.

There's also Tale of Two Wastelands which opens both Fallout 3 and NV up quite a bit - and makes 3 far, far more stable.

Just fair warning in that it can be a little bit of a pain to install sometimes.
