At restaurants, if you order an appetizer, does that actually slow down the meal? Sometimes I feel like it does, but I'm also often the one person who didn't order an appetizer, so it might just feel longer...

There are people (like my dad) who get really annoyed if the next course comes while they're still finishing the previous one, so a lot of wait staff will make sure to time the orders right so it doesn't feel rushed. I'm sure it feels like a long time if you're sitting there with no food in front of you.

You can always ask to get appetizers+main courses at once.

Nope. It doesn't. Appetizers are generally listed as such because they take less time to prepare than the mains. Regardless of whether or not you order one, your main is still going to take the same amount of time to prepare, and the amount of items on your ticket as it goes to the kitchen does not effect the the total amount of time to completion, as they start all items on a single ticket at the same time.
