I've had people startle me like that just by asking if I lost weight. I think each of the last few times I looked myself over before stammering "I don't know". I think I added a maybe afterward one time because I remember the person who asked adding "Is that a good thing?" which was awkward but at least she acknowledged that there are a lot of unpleasant things that cause people to lose weight.

Weight loss is an awkward thing to ask about in general imo. (What if the person has an ED, or its a sensitive topic for other reasons? Then you have people who haven't lost weight and don't want to.) Unless the loss is glaringly noticeable, I would never even consider mentioning it. Even then it's probably just better to say, “You look good".

You have a point about lots of unpleasant things causing weight loss. Most people jump to diet by default, and I guess that's because considering the other options is too... Well, unpleasant.
