I don't really see how drawing it a lot automatically equals to "fetishization"

Drawing someone with a skin disease "because it looks cool" is a type of fetishization, I would think.

Is it really a "disease" though?
Disease implies that it's hurting and/or killing the person who has it.
I don't think Vitiligo does that.

it's literally classified as a disease. a disease does not necessarily have to kill or physically harm someone, but vitiligo can certainly be argued to be psychologically harmful to the people who have it. a lot of people with it pay a shit ton of money for treatments (or use homemade treatments that are sometimes potentially dangerous) to try to lessen the appearance of their vitiligo because it takes a serious toll on their self-esteem.

People whose appearance is disconcerting are handicapped rather badly in society; you know that, right? People FLINCH from vitiligo sufferers in the real world. If you had a condition where you grew big, irregular warts on your face, would you say "It's not a disease! They don't hurt!"?

okay but like

this has been massive for years with other arresting visual quirks. Think of how many characters are drawn with eyes of two colours because it looks hella cool. I think vitiligo is better understood now so it crops up as an interesting quirk people imagine when humanising non-human characters, especially when the non-human character design has unusual markings of some kind.

ehhh when the people with vitiligo go through bullying and harassment because of their skin colors, and suffer psychologically and spend thousands of dollars on makeup and airbrushing to avoid the torment of living with their condition, it is pretty fucked up to reduce vitiligo to something that "just looks cool!" and force a trend about it. where were all these people saying how great and beautiful the folks with vitiligo were before it got popular on tumblr to draw every character with spots?
