It's not the only thing I wonder about, but yeah, same!

How do they deal with menstruation? With diarrhea?

I always like to think that the stress of living that far on the edge will cease menstruation.

And diarrhea...if in present times, I imagine anti-diaherreals...if in GoT style Medieval situation, most likely it will end with a tombstone, as you ain't gonna make it far with your entrails half way out your back side. AND if you're lucky, your water won't be contaminated, which is how I imagine you got sick to begin with...

I am pretty sure not everyone with diarrhea got it via the plague or dysentery.

Spoiled foods were more common than not. Rich people bought ice.

Speaking from experience- major stress can definitely delay or completely stop the mentruation cycle.

Boot camp for the Marines was the best 3 months of my life in that regard.

Panties are a very recent invention. Women wore skirts and blood and pee ran down and dried by themselves. When the floor was made of dirt, some drops of blood was probably of little concern.
