I must admit, when Jon Snow went down on Ygritte in GOT, that's pretty much exactly what I was thinking. Medieval cleanliness standards+hot winter clothing+sweating in combat=ew.

Though I'm guessing with stuff like Walking Dead they could still loot soap from supermarkets occasionally. That stuff lasts longer than canned food.

How do they deal with menstruation? With diarrhea?

Well, any fantasy setting involving magic, and I'm going to allow for the assumption for magical cleaning. Plus, in fantasy settings where you know there is water and bathing, I think cleaning is a possibility. Like, in Tolkien I imagine at least elves are rather fastidious about cleaning and probably have some form of soap. Men and dwarves probably less so. But often times LOTR or Hobbit fics will specifically mention the characters bathing in a river on the journey before engaging in sex. So authors do think about this.

Well, the human body regulates itself. If you stop taking showers and washing your hair the first days will be disgusting, but after some weeks the smell goes away and the hair stops being so greasy.

Ha, yeah this is always in the back of my mind as well, but I basically try not to think about it so I can enjoy the fic.

True. But then also remember that our modern sense of cleanliness is just that: modern. There are a lot of things we'd find irredeemably disgusting that, until relatively recently, were considered normal, or at least not so offensive as we'd find them now. Or simply things that people didn't talk about, even if they were bothered by them.

You stink, they stink, everyone stinks. People kept their genitals basically clean so they don't get sore and if they didn't fuck none of us would be here. People didn't do oral as much as we do now anyway. It's not something that would be expected unless you went and paid for it. And for the record I would wreck Daryl given half the chance and I don't give a damn what he smells like. All that manly smell would just turn me on.

TBH this is what I think about oral sex in modern day settings too. You meet some guy at a club at night, who hasn't showered since this morning and has probably gone to the bath room a couple of times during the day and you're supposed to put your mouth on his genitals? Eww.
