Well, any fantasy setting involving magic, and I'm going to allow for the assumption for magical cleaning. Plus, in fantasy settings where you know there is water and bathing, I think cleaning is a possibility. Like, in Tolkien I imagine at least elves are rather fastidious about cleaning and probably have some form of soap. Men and dwarves probably less so. But often times LOTR or Hobbit fics will specifically mention the characters bathing in a river on the journey before engaging in sex. So authors do think about this.
