TBH I can't even suspend disbelief that much. How can a world not ours magically have all the elements of Medieval Europe?

How can a world not ours have men and women, or people who breathe oxygen, or people with two arms and two legs, or sex, or romance, or brothers, or war?

I can't suspend disbelief for any fantasy because every single one has elements of our world in it.

I have more of that problem with scifi where there are too many Earth elements in other worlds. Scifi is supposed to be a more grounded genre. In fantasy, generally, you don't even know whether you're on a different world or an alternate one or what. It isn't as grounded in the same way, so I'm much more likely to be able to suspend my disbelief as long as the world is consistent within itself and sticks to its own rules.
