I am 100% certain OP knows what o-nii-chan means.

But incest plots or subtext are hardly rare in manga/anime. It's a common kink in that particular genre, and creators are known to deliberately stick in awkward scenes for fanservice, be it yaoi, incest, tentacles, whatever. This is when they AREN'T "canon" plot lines, like in Sword Art Online and other mainstream anime.

So yeah, a little sister screeching "o-nii-chan" and blushing a lot, could very well be an intentional incest hint.
