(and yeah I think a lot of people think about it, but I feel like it's usually not addressed just for simplicity's sake, especially in a show that's not supposed to be super gritty)

Pretty much. The only time I can recall being bothered by no one going to the bathroom was during the first season of 24, and that's just because of the premise.
