I personally use Sai for 99% of the process, and then do final adjustments like adjusting saturation or adding motion blur or textures in photoshop.

I don't personally use premade palettes. Sometimes I'll dig up a classic painting and pick from that.

And for music, I have a lot of private youtube playlists of various kinds, plus lots and lots on my iPod. I mostly listen to non-vocal pieces like soundtracks or different electronica genres, so as to not divide my attention between lyrics and painting.

Sai's pretty good for most of it. Sometimes I use Manga Studio 5 for coloring as well.
