All those wild, fantastical adventures are very much the stuff of dreams. And the companions being nurses makes so much sense.

Even if I personally prefer when you switch between dream and reality and don't know which is which. But this would make a delightful short film, especially if it starred Peter Capaldi.

I genuinely don't get why Doctor Who would be more interesting your way. I find going away to adventures much more fun.
OTOH, I did want the Buffy-as-a-mental-hospital-patient to be the last episode of Buffy the Vampire Slayer...
