i genuinely have no understanding at all of the kind of perspective you're coming from, or the kind of person you are, if you think this is a more interesting, better interpretation of Doctor Who.

I'd like it if there were little side scenes where the family/nurses all kind of go, "another 'adventure' again? okay, you be the aliens this time" or something.

There might be something you can't have this head-canon about, but 'it's all a dream' is an old plot twist and could work with most things. Rarely have I found it to make events more interesting than whatever initially seemed to be going on (especially in the case of DW, though I of course have my issues with it), but different strokes.

I really love this idea, OP! I don't watch Doctor Who, and I'm not saying this would be my headcanon if I did watch the show. I don't even know if I would want to read a fic of this idea, because I think it would be a real downer. But just as an idea itself, I find it completely charming and incredibly fitting.

I feel like the people who are saying you could apply this scenario to any story are technically correct, but are missing how well it fits with Doctor Who in particular. All those wild, fantastical adventures are very much the stuff of dreams. And the companions being nurses makes so much sense.

That is one of the theories that always make no sense to me. I mean you might as well think that it was all made up by writers trying to make a TV show.

It may be due to overexposure. But recently I find the "all was just a hallucination"-trope a bit boring.

But hey, if your interpretation of the story makes you happy, that's a good thing.

The Eighth Doctor book "The Blue Angel" sort of includes this angle.

I find the concept of this headcanon rather sweet, and I for one am not exhausted by the It's All Just A Dream trope (likely because I never see it used). Even if I personally prefer when you switch between dream and reality and don't know which is which. But this would make a delightful short film, especially if it starred Peter Capaldi.

How do you explain all the incarnations?

I genuinely don't get why Doctor Who would be more interesting your way. I find going away to adventures much more fun.

OTOH, I did want the Buffy-as-a-mental-hospital-patient to be the last episode of Buffy the Vampire Slayer...
