I really love this idea, OP! I don't watch Doctor Who, and I'm not saying this would be my headcanon if I did watch the show. I don't even know if I would want to read a fic of this idea, because I think it would be a real downer. But just as an idea itself, I find it completely charming and incredibly fitting.

I feel like the people who are saying you could apply this scenario to any story are technically correct, but are missing how well it fits with Doctor Who in particular. All those wild, fantastical adventures are very much the stuff of dreams. And the companions being nurses makes so much sense.

Now that you mention it, yes, it does fit with Doctor Who extraordinarily well. A lot of the plots have that Walter Mittyesque, Baron Munchausen-like quality.
