From what I can tell, on Tumblr anyway, it's not deep thought out thing. It's very simple. They see a girly boy or a not so girly girl and assume that the girly boy is a transman and the not so girly girl as a transwoman.

That's it. Or, if they are rather androgynous, but have SOME SMALL characteristic that leans one way or the other, that's the headcanon. Look at that one female character in AoT awhie back. She likes potatoes as I can't remember her name.
