it's an illustration from a fic. He's having some kind of severe muscle spasm (side-effect of cruciatus I believe). The "Hermione as Snape's healer" thing is kind of popular among shippers.

(I have to say, the fic in question is not all that Lily Fox cracks it up to be -- I couldn't get through it myself because HG is just a bit too naive and eager-beaver-y.)
