Alternately, calling a character by their given name when it's not (or rarely) used in canon. If someone other than Nana refers to Hardison as "Alec" in your Leverage fic, I'm out.

The ONLY time I am okay with that is for characters that get into a romantic relationship. I sort of assume characters that move from a work/friendship to romantic relationship to use more intimate names.

This brings me back to my years in the X-Files fandom. Scully calling Mulder "Fox" was a deal breaker basically 100% of the time, and for probably 99% of the fandom. Every once in a while you'd come across a fic where Scully would call Mulder "Fox," and that's how you knew you were reading an incredibly bad shipper fic that basically made the characters into generic Ken and Barbie. It was an instant "bail out" signal.

Mulder calling Scully "Dana" was not quite so bad, since Mulder does actually use her first name occasionally in the show.

This is me with Sherlock Holmes fandom and fic using Holmes & Watson's first names that isn't set in the BBC series or Elementary. Because no.
