I want to believe in 1 and have met a few people capable of 2, so I'm guessing 3 is the lie.

Being able to lick your elbow is not that rare. I can do it, and so can a bunch of people I know.

I say 2 is a half-lie, and that you can lick your elbow, but it's not due to a medical condition.
