None. :( I've only ever seen pharmacy in shows/movies be the people responsible for fucking up a patient's meds. It's NEVER the nurse or the doctor. (When it usually IS the nurse or the doctor).

haha, I work in a lab and have a similar experience. I used to work in the part of the lab that handles blood/urine/etc. testing. Nurses fuck up blood tests all the time (not enough blood, wrong container, wrong transport temperature, etc.). This is usually not a big deal (unless the patient is a hard stick) because it's like 5 mL of blood and you can just get more. But apparently some nurses will tell the patients the lab screwed it up and that's why they have to get stuck again. :/
