Raziel's death in Legacy of Kain: Defiance (and also his sorta death at the beginning of Soul Reaver).

Arthur's death on Merlin.

Aeris's death. :(

And not so much the death scene itself, but Harry's grief and anger at the end of OoTP, first in Dumbledore's office and then the scene with Luna.

Seconding Aeris's death. it was really beautiful and chilling, and I love that she didn't have any "last words", that would've been too cheesy.

So much agree on OotP. That scene literally changed me as a kid (I read it when I was 11). It made me feel and understand grief and death like nothing else - I had never experience the death of a close friend or family member at that time. It still gets to me every time.
